Insurance Plans
Eligibility for insurance(s)/benefits is for full-time employees - based on 40 hours per week. (Hire date + 30 days to first following month).
******************YOU HAVE ONLY YOUR FIRST 30 DAYS TO ELECT COVERAGE***************** Principal Financial Group provides Medical, Dental, Vision and Short Term Disability as a package. PHCS is the PPO where you pay $25 co-pay for PPO listed physicians/suppliers - 60%/40% for a max of $1,000 (individual) and $2,000 (family) for outside network. Florida Health Care Plan (FHCP) provides Medical and limited Dental only. FHCP is an HMO. You must use providers/physicians on the FHCP list.
